Coconut milk does not occur naturally in liquid form, and is different from coconut water which is found in green coconuts.

Coconut milk is traditionally made by grating the white inner flesh of a coconut which then provides a liquid. This liquid is combined with water in order to obtain the desired percentage of fat. The rich taste and creaminess of coconut milk is due to its high oil content.
